Overview
Self-hosted Cal.com replaced a WhatsApp-based booking chaos across 7 clinics and 38 physicians. Custom appointment types map to Kuwaiti MOH specialty codes, Arabic RTL UI skins match the group's brand, and an n8n webhook fires patient reminders via WhatsApp and SMS. No-show rate falls from 31% to 9%; admin headcount for scheduling drops from 6 to 2.
The challenge
Patients booked via seven separate WhatsApp numbers; doctors missed slots due to no single source of truth. MOH rules required specialty-coded appointment records for audit. The group also needed Arabic-first UI as the majority of their patient base does not read English.
The solution
We extended the open-source Cal.com codebase with a custom MOH-specialty field and an Arabic RTL theme layer. The booking flow was self-hosted on a Kuwaiti VPS to keep patient data in-country. An n8n workflow listens to Cal.com webhooks and dispatches bilingual WhatsApp and SMS reminders at 24h, 2h, and 30-min windows, with a one-tap confirm/cancel response that writes back to the booking record.
